הצגת מקרה - מרדכי ז"ל

למרות שניתן להיעזר בהם רבות, שימוש בהנחיות מקצועיות ותרשימי זרימה של מערכת הרפואית, ואף הנחיות מקדימות של המטופל מהוות תמיכה חלקית בלבד כאשר מדובר בהתלבטויות מקצועיות המתרחשות בסוף החיים. דילמה שכיחה היא התהייה היא במיוחד במצבי סוף החיים כיצד לפעול כאשר המצב הרפואי והכללי של המטופל מחמיר ומתדרדר ולא קיים טיפול רפואי אפקטיבי. בפרזנטציה הוצג מקרה של מרדכי, אדם אינטליגנטי, דעתן, ניצול שואה, חבר קיבוץ בעל משפחה תומכת וקרובה. המקרה הוצג ע"י ראיון עם בני המשפחה – מה שאפשר למשתתפים לחוות את חווית הדילמה דרך עייני בני המשפחה המסורים. מרדכי אובחן כחולה אלצהיימר. עיתוי הסיפור מתרחש אחרי שניצל באשפוז בבית הולים. דווקא מיד אחריכן עמד על כך שלהבא הרופא יבטיח לא להפנותו שוב לבית החולים יהיה מה. בראיון ובדיון לאחר מכן הוצגה גישתו של הרופא לטיפול בחולה ולמשפחתו בהמשך. סוגיות שעלו כללו בירור מה מטרת הטיפול, באיזו במידה תפקידו של הרופא הוא לקבל החלטות רפואיות בלבד מול העצמה של החולה או תפקידים נוספים כגון ליווי המשפחה. מוטיב נוסף בפרזנטציה התייחס לחשיבותו של "התהליך" ולא רק התוצאה.

Abstract
Despite the use of guidelines and algorithmic thinking together with advanced directives, the physician decision-making process in end of life situations occurring in nursing homes is not straight forward. While there are situations where the decision tree is self-evident, I have found that reality can confound this theoretical clarity. The uncertainty factor and the inadequacy of the tools available may simply be too great. A typical dilemma can occur when the patient’s condition deteriorates and the goal of treatment is to relieve suffering. Under what conditions, if at all, should the patient be referred to hospital? It may be challenging for a physician to successfully assess let alone effectively plan relief of suffering in an uncommunicative patient. After all in contrast to pain, suffering assessment is not an automatic part of a physician's purview. The gap between wanting and successfully alleviating suffering can be great; the present theoretical tools are often inadequate. In my presentation I will attempt to demonstrate that while there is not a recipe, there is a preferred approach: The physician can use relevant guidelines and directives coupled with an ongoing physician led process which includes relevant patient, family and team member involvement which can often lead to an acceptable and accountable plan of action The presentation will include a taped interview with the family of a resident that our nursing home took care of. The expected outcome is that the ensuing discussion will shed light on the difficulties physicians and other caregivers encounter and see if the model presented can be of assistance.
